Part-Time Employee Status Form

    • The Employee Status form for the Fall 2009 semester is due in the Human Resources Office by 8/16/09.
    • The Employee Status form is NOT required for the Summer semester unless you are a Sac State student working less than 30 hpw and attending summer school at Sac State.
    • Students must also submit a copy of their class schedule. The form is available online and can either be faxed, mailed or hand delivered to the UEI HR office. The form is a requirement of continued employment.
