Step 1: Notify the Risk Management department that your employee needs a new chair.
Step 2: The Risk Management department will provide you with a letter for the employee.
Step 3: Give the letter to the employee who is requesting a chair.
Step 4: The employee should call “The Chair Place” to set up an appointment.

Step 5: The owner of “The Chair Place” will assist the employee in their selection of a chair. The decision of the color and fabric is up to the department.
Step 6: Once the selection has been made, the employee will return to the workplace with a written quote.
Step 7: The employee’s manager or supervisor should make a copy of the quote and send it to the Risk Management department.
Step 8: Each department/project manager and supervisor is responsible for completing a purchase order, using the information on the quote.
Step 9: The purchase order should be faxed to “The Chair Place”. The fax number is (916) 446-7728.
Step 10: A copy of the P.O. should be sent to the Risk Management department.
Additional Information:
- It takes 3-4 weeks for the chair to be delivered.
- Contact the Risk management department when the chair is delivered.
- If after 4 weeks the chair has not been delivered, please contact the Risk Management department.
