Request CDD.net User ID

All information described below must be provided
unless otherwise stated.

Requestor to provide:

1. Your full name (first and last)
2. Your title/position
3. Your department name
4. Your work phone number
5. Type of Access Requested: University Foundation at Sacramento State GL Reporting
6. Your email address
7. Your preferred User ID (optional, see below)
8. E-mail the above information to your Manager/Supervisor requesting approval.
9. Instruct your Manager/Supervisor to forward your request with his/her approval to tfgca@fndmail.csus.edu

Manager/Supervisor to provide:

1. State that you authorize the Requestor access to the University Foundation at Sacramento State GL Reporting.
2. Include your name, title and phone number.
   
3. Forward the e-mail with all information (Requestor and Authorization) to tfgca@fndmail.csus.edu.

1. Must be 4 to 8 characters in length.
2. Contain only letters A-Z or numbers 0-9. There is no difference between upper and lower case (i.e: b is the same as B).
3. No spaces or special characters.
4. Can be but does not have to be the same as your CMS user ID.
